Section 26(1) in The Orissa Ayurvedic Medicine Rules, 1960
(1)A certificate to be signed or authenticated by the registered Ayurvedic practitioner under Clause (c) of Sub-section (3) of Section 39 shall contain among other particulars necessary for the purpose, the name and address of the patient, age and the nature of ailments or details of the injuries so certified.
