Section 4(1) in U.P. Kshettra Panchayats (Removal of Disqualification and Settlement of Disputes Relating to Disqualification and Membership) Rules, 1994
(1)An application for the removal of the disqualification under clauses (c), (e), (f), (g), (h), (i), (j), (k) or (l) of Section 13 of the Act, shall be in Form I and shall show the grounds upon which the applicant claims removal of the disqualification.
