Section 10(2) in M.P. Rights of Persons with Disabilities Rules, 2017
(2)The records shall contain the following particulars, namely :-(a)Number of persons with disabilities who are employed and date of their appointment.(b)The name, gender and address.(c)Nature of disability.(d)The nature of work which is being performed by such person with disability, and(e)The details of facilities which is being provided to such persons with disabilities.
