3. Forms of applications and licences.
- Every application for a licence specified in column 1 of the table below shall be made in the Form specified against it in column 2 of the said table and the licence, when granted, shall be in form specified in the corresponding entry in column 3 thereof.| Nature of licence | Form of application | Form of licence |
