Section 33(12) in Orissa Minor Minerals Concession Rules, 2004
(12)As soon as removal of the mineral granted under this permit is completed, the permit holder shall surrender the permit to the competent authority and furnish to him particulars of permit in FORM-U indicating the quantities of minor minerals removed and other information as may be required by the competent authority.
