(a)As each case is instituted, the clerk-in-charge of the record shall prepare a General Index in (F. 4) which shall be prefixed to the record of every case and each paper as it is filed unless otherwise directed in these rules shall be entered in the index, along with its dale of presentation, in the order in which they were brought on record.
