(1)The statements shall be furnished by every patentee and every licencee under sub-section (2) of section 146 in [Form 27] [ Substituted by S.O. 1418(E), dated 28.12.2004, for " Form 29" (w.e.f. 1.1.2005).] which shall be duly verified by the patentee or the licencee or his authorised agent.
