Section 43(2) in Rules under the Pharmacy Act, 1948
(2)Every application for change of name or surname shall be made to the Registrar by the registered pharmacist himself, and shall be accompanied by the fee prescribed for such application in Rule 47 also by an affidavit sworn by the registered pharmacist before a Magistrate stating that the applicant is the same person whose name is registered with the particular number, and the circumstances for which the change is sought.
