Section 98(16) in The U.P. Minor Minerals (Concession) Rules, 1963
(16)The licensee shall without delay, send to the District Officer a report of any, accident, causing death or serious bodily injury or serious injury, to property, or seriously affecting or endangering life or property, which occur in the course or any operations under the licence.
