B- Among them, as per "A"(above) information the names of the officials whose LTC bills/ Medical reimbursements/bills/cases had already cleared, their names, date wise serially with amount.
C- Amount them as per "A" information the name of the officials whose LTC bills/ Medical reimbursements/bills/cases are yet to be cleared with amount date wise and serially (i.e. pending cases).
D- The pending cases (unclear) as per "C" information, reason of pending, be stated against the pending staffs.
E- The name, address, phone number, email ID, fax no. of the appellate authority, Kolkata, relating the above said matter with designation.

The following were present Appellant: Mr. Arabinda Majhi on telephone through mobile no. 09038047173; Respondent: Mr. S. K. Ghose(09830403290),CPIO on video conference from NIC-Kolkata;
The appellant stated that he wants the following information relating to Salt Lake Office of EPFO for the period 2009 to 2012:
1- Name of officers and the amount of LTC and medical expenses reimbursed to them. 2- Name of the officers whose bills for LTC and medical expenses are pending and reasons thereof.
The CPIO stated that he will furnish the information.
Decision notice:
As stated by the CPIO he should furnish the information as above to the appellant, free of cost, within 21 days from the date of receipt of this order.
The appeal is disposed off accordingly.
