An application for a direction, order or writ under Article 226/227 of the Constitution shall be moved in the format prescribed in three sets duly supported by an affidavit by the petitioner or by one of the petitioners or by some other person proved to the satisfaction of the Court to be acquainted with the facts of the case as specified in sub-rule (1) of Rule 15 of Order VI of the Code. It shall also state whether an application on the same facts had been previously filed before this Court, and if so, with what result.
