Section 5 in The Karnataka Private Medical Establishments Act, 2007
5. Application for Registration.
Every person desiring to establish, run, maintain or continue to run and maintain a Private Medical Establishment shall make an application to the concerned Registration Authority in such form, in such manner and along with such fees as may be prescribed and different amount of fees may be prescribed, for different class or classes of Private Medical Establishments.
