(1)Every person who desires to have his name registered in the register shall submit to the Registrar an application in the prescribed form stating:(i)particulars of his qualification;(ii)the period for which he has been in practice;(iii)his date of birth to verify the attainment at minimum age-limit; and(iv)his address which is to be his registered address.
