Section 94(2) in The Orissa Motor Vehicles Rules, 1993
(2)Such complaints shall be written clearly and in an intelligible manner in English or any of the language of the district and the complaints shall also clearly and legibly record in the complaint book his full name, address and date on which such complaint are written.
