Section 68(13)(a) in The Bihar Co-operative Societies Rules, 1959

(a)Reference of a dispute to the Registrar shall be made in Form no. XII, application for decision shall be made in Form no. XIII, notices to parties shall be issued in Form no. XIV and summons shall be issued in Form no. XV.
