(4)A copy of letter in Form 3 addressed to Superintendent of Government Hospitals/Chief Medical & Health Officer as the case may be shall be endorsed to the applicant and the [Director, Pension Department] [Substituted vide F. D. Notification No. F. 1 (45) FD/Gr. 2/83, dated 17-1-1984 w.e.f. 1-12-1983.] by the Head of Office.
