Section 16(1) in The Orissa Government Securities Rules, 1938
(1)Every application regarding a treasury bill alleged to have been lost, stolen or destroyed, either wholly or in part, shall be addressed to the office of the Bank which issued it, and shall be accompanied by a registration fee of Re. 1 per treasury bill and a statement of the following particulars, namely :(i)particulars of the number and value of the treasury bill;(ii)the circumstances attending the loss, theft or destruction; and(iii)whether the loss or theft was reported to the police.
