Section 5(1) in The M.P. Maternity Benefit Rules, 1965
(1)The fact that a woman is pregnant or has been delivered of a child or has undergone miscarriage or is suffering from illness arising out of pregnancy, delivery, premature birth of child or miscarriage shall be proved by the production of a certificate to that effect, from the registered medical practitioner. The certificate shall be in Form C.
