Section 8(1) in The Sailing Vessels (Members Of Crew) Rules, 1967
(1)Where a permit or an identity card is lost destroyed, defaced or mutilated, the holder thereof may apply, along with an affidavit setting out the facts, to the Issuing Authority for a duplicate copy thereof.
