Section 75(3) in The U.P. Narcotic Drugs Rules, 1986
(3)The Medical Board shall forward its recommendations to the Collector of the district concerned on Form NDO-4 alongwith other records connected with the case. The record of the medical examination shall be maintained by the Medical Board in a register in Form NDO-5.
