(1)A brief report (Form No. 148) on every escape that has taken place, shall be submitted to the Inspector-General at once, to be followed by a full report detailing the results of the enquiry which the Superintendent shall make as soon after the occurrence as possible. A copy of the judgment in the case of a prisoner tried for escaping, shall also be submitted to the Inspector-General. In the case of escapes that have not been due to negligence alone but in part to some defect in the buildings or in the method of guarding, such defect should be clearly pointed out.
