(1)An application of the grant of conductor's licence as required by sub-section (1) of section 30 of the Act shall be made in the Form, L.Con.A and shall be accompanied by a medical certificate in Form M.C.Con., valid adult First Aid certificate from St. John Ambulance Association (India) and the fee as prescribed in sub-section (5) of the said section 30.
