Section 21(b) in United Provinces Medical Act, 1917
(b)if he is registered under the Medical Acts, -(i)must correctly inform the Registrar of the date of such registration, and(ii)must furnish the Registrar with a correct statement of the titles or qualifications in respect of which he is so registered, and of the dates on which he obtained them, or
