Section 17(2)(f) in The Punjab Factory Rules, 1952

(f)When a person to whom a certificate under section 69(2) has been granted loses such certificate, he may apply to the Certifying Surgeon for a copy of the certificate, and the Certifying Surgeon, after making such enquiry from his employer or if he is unemployed from his last employer and from other sources as the deems fit, may grant a duplicate thereof. The word "Duplicate" shall be clearly written in red ink across such certificate and initialled by the Certifying Surgeon. The counterfoil in the bound book of forms shall be similarly marked 'Duplicate' and initialled.
