(2)Such petition shall,(i)be signed by the prisoner and attested by the Jailor,(ii)be accompanied by a nominal roll of the prisoner in Form No. XVII and(iii)subject to the provisions of rule 340 be forwarded by the Superintendent with a covering letter addressed to the Secretary to the Government in the Home Department.
