(4)The declaration under sub-section (1), and an additional declaration under sub-section (2), shall be in the prescribed form, containing the prescribed particulars. and [shall (together with the certificate of taxation) be delivered] [These words and brackets were substituted for the words 'shall be delivered' by Maharashtra 43 of 1969, Section 3(a).] after being duly filled up and signed, within the prescribed time. The additional declaration shall indicate clearly also the nature of the alteration made in the motor vehicle, or as the case may be, the altered use to which the vehicle is proposed to be put.
