- A licence-holder shall maintain a register containing duplicate copies of all prescriptions which contain any of the poisons mentioned in Schedule I. This register shall contain the following particulars:-(a)Serial number(b)Date(c)Name of prescriber (Doctor)(d)Name of person for whom prescribed (patient)(e)Copy of prescription(f)Detail of poison in the prescription: (i) Name of poison (ii) Total quantity.(g)Signature of dispensor.
