Section 78(a) in The U.P. Co-operative Societies Rules, 1968
(a)A nomination under Rule 77, shall be made by a member by signing a declaration on the form prescribed by the society or by making a statement in any book kept for the purpose by the society. When a nomination is made by signing the declaration, such declaration shall be deposited with the society during the member's life time. In either case the nomination shall bear the signature of the member making nomination and shall be attested by two witnesses.
