- The Medical Officer shall keep a special record of cases of cholera and at the close of an outbreak, write a report on the origin and progress of the disease with the measures taken to combat it and any points of interests that came under his observation. Copies of the record and report in triplicate shall, if the disease has been epidemic, be forwarded at once to the Inspector-General, one for his own use and the other two for transmission to the Director of Health Services with the Government of India, respectively. If the disease has been limited to a sporadic case or two, copies of the record and report shall be submitted in triplicate at the close of the year in which the disease occurred.
