Section 15(2)(a) in Rajasthan Government Servants' General Provident Fund Rules, 1997

(a)The account holder shall apply to his head of office for temporary withdrawal in the form prescribed by the Director. He shall enclose the pass book duly completed by the Drawing and Disbursing Officer and verified by Department along with application.
