Section 21(5) in Chhattisgarh Council for Physiotherapy and Occupational Therapy Act, 2015
(5)The register shall be in such form and may be divided into such parts as may be prescribed, which shall include the name in full, address, date of birth and the qualification of registered physiotherapist and occupational therapists, the date on which the qualification was obtained, and such other particulars as may be prescribed.
