(4)Application for a licence shall be made in the Form provided in Appendix I and addressed to the Inspector having jurisdiction over the place from or to which the animals or any product or carcass of any animal or any fodder, bedding or other thing used in connection with the animal concerned is/are proposed to be brought, removed or carried.-
