Section 42(2)(d) in The Sakri Canals Irrigation Rules, 1952

(d)be in the Form prescribed by the State Government which shall be supplied free at all canal offices, and shall state clearly the name or the names of the persons objecting, the village and canal on which the fields are situated, the crop and the year for which the charge is made and the demand from each occupier to which objection is taken.
