(2)The application for the amendment of a licence shall be accompanied by,-(i)a statement indicating the nature of the amendment required;(ii)a statement showing the change in particulars already furnished in Form I under rule 3 which necessitate the application for amendment;(iii)fresh plans as provided in rule 3 in cases where the plans submitted with the original application no longer hold good;(iv)a treasury receipt showing that the appropriate fee for the amendment of licence has been paid in the local treasury; and(v)the licence originally granted.
