Section 13(2) in Goa Lotteries (Regulation) Rules, 2003
(2)A application claiming all taxable prizes shall be made to the Director in Form I hereto by the claimant alongwith the genuine prize winning ticket, two passport size photographs, domicile certificate and any other details as required by the Director.
