(3)Where the applicant has branch offices outside the jurisdiction of the Licensing Authority, a separate application shall be made to the Licensing Authority in whose jurisdiction the branch office situated in Form M.RM.V.R.-61 (PAGA) accompanied by a fee as specified in Rule 145.
