Section 68(1) in Rajasthan State Dental Council Rules, 2008
(1)A registered person may himself make an application for the removal of his name from the register. The application shall be accompanied by a declaration made by the applicant, that he is not aware of any proceedings or reasons for the removal of his name from the register or in depriving him of a licence entitling him to be registered.
