45. Application for duplicate certificate.
- Application for a duplicate certificate shall be made to the Chairman with an affidavit attested by a Gazetted Officer or a Magistrate stating that the certificate granted under these rules, has been lost, stolen, destroyed or mutilated without any fault on the part of the applicant.
