(3)Every application for registration shall be accompanied by the rules and the bye-laws of the Association, the names and addresses of the office bearers and the members of the executive committee of the Association and an up-to-date list of the members of the Association showing the name, address, age and date or year of enrolment of each such member and the place where he ordinarily practises.
