(2)Every self- declaration or affidavit under sub rule (1) shall be -(i)in the form 4-B-1, in case of election of Sarpanch;(ii)in the form 4-C-1, in case of election of Member of Janpad panchayat;(iii)in the form 4-D-1, in case of election of Member of Zila panchayat.
