(i)keep a book readily accessible (hereinafter called "the Accident Book") in [Form 11] [ Substituted by Noti. No. N-11/13/2/2003-P&D, dated 1.10.2004, for " Forms 15" (w.e.f. 1.1.2005).], in which the appropriate particulars of any accident causing personal injury to an insured person may be entered;
