Section 37P(v) in The Narcotic Drugs and Psychotropic Substances (Madhya Pradesh) Rules, 1985
(v)The Medical Officer shall prepare a record of the medical examination in triplicate in Form P.S. IX and shall give one copy to the applicant while forwarding one copy to the District Excise Officer concerned and retain one copy for the permanent record of his officer.
