Section 5(1) in The Orissa Stage Carriages (Licensing of Conductors) Rules, 1994
(1)An application for the grant of a conductor's licence as required by Sub-section (1) of Section 30 shall be made in Form II and shall be accompanied by a medical certificate in Form III and the fee specified in Sub-section (5) of Section 30.
