24. Form of application.
- The application shall be made in the form approved by the Commission/Board or the Appointing Authority, as the case may be and obtainable from the office of the Commission/Board or t he Appointing Authority, as the case may be on payment of such fee as the Commission/Board or the Appointing Authority may, from time to time, fix.
