(2)A separate certificate in quadruplicate will be prepared by the Compensation Commissioner in A.I.Form 12 showing the amount of each annual instalment and the interest thereon and the date on which and the treasury or the sub-treasury at which the annual instalment will be payable to the Guzaredar under sub-section (2) of Section 16.
