Section 20(2) in The Post Office Savings Certificates Rules, 1960
(2)Every application be accompanied by:-(a)a statement showing particulars, such as, numbers, amount and date of the certificate and the circumstances attending such loss, theft, destruction, mutilation or defacement.(b)identity slip, if any;
