12. Issue of a duplicate Certificate of Registration.
(1)In case a Certificate of Registration granted to a TPA by the Authority is lost or destroyed or mutilated, the TPA shall submit to the Authority an application for issue of duplicate Certificate of Registration in such form as may be specified by the Authority.(2)Such application shall be accompanied by a processing fee of rupees two thousand and applicable service tax and the same shall be paid to the Authority in the manner mentioned in Regulation 7(2) of these Regulations.(3)The Authority, on being satisfied that the original Certificate of Registration has been lost, destroyed or mutilated, shall issue a duplicate Certificate of Registration to the TPA in the form as specified by the Authority.
