Section 11(1) in The Chhattisgarh Motor Vehicles Rules, 1994
(1)If at any lime a driving licence is lost destroyed or mutilated the holder shall forthwith intimate the fact in writing and may apply for a duplicate in Form C.G.M.V.R.-3 (LLD) to the Licensing Authority in whose area he has his place of residence at the time.
