(c)There shall be sent with the corpse -(i)a chalan in West Bengal Form No. 5371 ;(ii)a disposal order in West Bengal Form No. 14279;(iii)the Investigation (or inquest) Report;(iv)a request to the Medical Officer for examination in West Bengal Form No. 4267 (in the Town of Calcutta);(v)a request to the Coroner in West Bengal Form No. 4268 (in the Town of Calcutta);
