Section 111(1) in The Bihar Motor Vehicles Rules, 1992
(1)An appeal or a revision under Chapter-V of the Act shall be preferred in duplicate, in the form of a memorandum, setting forth concisely the grounds of objection to the order against which the appeal is preferred, and shall be accompanied by a certified copy of that order and a fee prescribed under Rule 74.
